Minor Consent for Treatment at Medical Services
Under Colorado law, students under the age of 18 who receive medical treatment at Wardenburg Health Center must first have a parent or guardian sign the Parental/Guardian Consent for Treatment of a Minor form*.
*Please see exceptions below for minors consenting to healthcare services.
How to submit a Parent/Guardian Consent for Treatment of a Minor
Students can access and complete the Parent/Guardian Consent for Treatment of a Minor form on MyCUHealth.
If a parent or guardian is present, you can submit the consent form through your online health portal.
- Sign into your using your CU IdentiKey and password.
- On the main page, select the option for Medical Services at Wardenburg.
- On the Medical Services page, click on Forms in the left hand menu.
- Scroll down to the section labeled Wardenburg Medical Services Forms.
- Complete the online form and submit.
If a parent or guardian is not present, you can give them a hardcopy of the consent form to fill out. You can then upload the completed document to your student health portal or send it directly to Medical Services.
- Download a Parent/Guardian Consent for Treatment of a Minor form to give to your parent or guardian to complete.
- Once the form is complete and signed, you can upload your document online.
- Sign into your using your CU IdentiKey and password.
- On the main page, select the option for Medical Services at Wardenburg.
- Select Uploads from the left hand menu.
- Scroll to the section labeled Medical Documents.
- Upload your signed consent form and click Submit.

Minors Consenting for Their Own Treatment
Minors can consent to certain medical treatments if parental consent may impact their ability to seek those treatments, including:
- Pregnancy-related care, contraceptive care or abortion procedures
- Care for sexual offenses or assault
- Sexually transmitted infection care
- Substance use disorder treatment or mental health services
Colorado law also allows minors to consent to their own medical care of any kind if they are 15 years or older, are living apart from their parents and managing their own financial affairs, regardless of the source of their income.
Students under 18 years of age who wish to consent for their own treatment at Medical Services must submit the Authorization for a Minor Consent for Their Own Treatment form.
- Sign into your using your CU IdentiKey and password.
- On the main page, select the option for Medical Services at Wardenburg.
- On the Medical Services page, click on Forms in the left hand menu.
- Scroll down to the section labeled Wardenburg Medical Services Forms.
- Click the link for Authorization for a Minor Consent for Their Own Treatment.
- Complete and submit the form.
