Complex Care Team: Continuity of Care

Our team is made up of medical providers, nurses, behavioral health providers听and most importantly, YOU.

We are dedicated to working with you and your specialist(s) to ensure continuity of care for the management of your complex听medical condition while you are a student at CU 麻豆影院. As a team, we are committed to empowering and educating you on how to take the lead in managing your own health.

Scheduling an appointment

Eligibility

All CU 麻豆影院 students can access the Complex Care Team.

Ongoing visits with the CCT are covered by听Anthem Gold SHIP.

If you elect to use your own private insurance, we can bill them directly. Simply provide us with your insurance card when you check in for your appointment.听

The听BuffCare Supplement听can help cover any out-of-pocket expenses after the insurance has been billed.

Prepare for your visit

To prepare for your appointment, please review the听Welcome Packet听including signing the MOU and sending your specialist the Specialist鈥檚 Information Attachment and Medication Order Attachment to complete and sign.

Schedule an initial 40-minute appointment with the Medical Clinic by calling 303-492-5432.

Initial appointments can be scheduled prior to the start of the academic year. They are a great way to discuss current medical conditions, review any lab and paperwork from previous providers听and can include ordering new lab work and establishing a treatment plan going forward.
