Name of former reviewer (e.g., John Doe).
Name and IdentiKey of new reviewer (e.g., Jane Doe, jado1234).
Identify where this reviewer should appear in the workflow (e.g., "after Jane Doe and before Ralphie Buffalo").
Starting with the first reviewer in the workflow, provide each reviewer's full name and IdentiKey, if known.
Identify the catalog pages this person should review (e.g., "all Math Department majors, minors and certificates" or list page URLs).
Identify the catalog pages the workflow applies to (e.g., "all Math Department majors, minors and certificates" or list page URLs).
Upload any supporting documentation that will help us complete your request (e.g., a revised Excel spreadsheet with highlighted changes, a Word doc with tracked changes).
